چکیده

2 Department of Orthopaedic Surgery, Columbia University Medical Center, New York, NY The glenohumeral joint is one of great mobility facilitated through the complex interplay of soft tissue and osseous anatomy. Arthroscopic shoulder stabilization has become the standard of care in the surgical management of glenohumeral instability. However, the management of the unstable shoulder associated with a bony defect (glenoid, humeral or combined) can be challenging and preclude arthroscopic treatment. Adequate diagnosis of bony defects is paramount to successful treatment and entails a careful history, clinical exam, and specific radiographic imaging. In general, higher energy shoulder trauma leads to more significant glenoid and/or humeral head defects. In addition, the severity of these defects corresponds with the number and frequency of instability episodes. Non-operative methods of treatment are not sufficient for treating these cases. Although successful arthroscopic management of instability associated with osseous defects has been described, open reconstruction is often indicated.

A Theoretical Model of the Effect of Bone Defects on Anterior Shoulder Instability

INTRODUCTION: The presence of either a Hill-Sachs or a bony Bankart lesion has been indicated as a possible cause of subluxation and anterior shoulder dislocation. Anterior dislocation accounts for approximately 98% of all shoulder dislocations. 2 Common treatments include soft tissue repair, bone grafting, and total shoulder replacement. It has been reported that there is a possibility of recu...

Results of Open Bankart Surgery for Recurrent Anterior Shoulder Dislocation with Glenoid Bone Defect and Concomitant Hill-Sachs Lesion

Background: Open Bankart surgery is the main treatment procedure in patients with recurrent anterior shoulder dislocation, especially in cases with glenoid bone defect. The goal of this study was to determine the outcomes after open Bankart surgery in cases of recurrent anterior shoulder dislocation with glenoid bone defects and concomitant Hill-Sachs lesions. Methods: Between 2006 and 2010,...

Risk factors which predispose first-time traumatic anterior shoulder dislocations to recurrent instability in adults: a systematic review and meta-analysis

BACKGROUND Recurrent instability following a first-time anterior traumatic shoulder dislocation may exceed 26%. We systematically reviewed risk factors which predispose this population to events of recurrence. METHODS A systematic review of studies published before 1 July 2014. Risk factors which predispose recurrence following a first-time traumatic anterior shoulder dislocation were documen...

The Hill-Sachs lesion: diagnosis, classification, and management.

The Hill-Sachs lesion is an osseous defect of the humeral head that is typically associated with anterior shoulder instability. The incidence of these lesions in the setting of glenohumeral instability is relatively high and approaches 100% in persons with recurrent anterior shoulder instability. Reverse Hill-Sachs lesion has been described in patients with posterior shoulder instability. Gleno...

Determination of prevalence of glenoid bony lesions after recurrent anterior shoulder dislocation using the 3-D CT scan

Background : Glenoid bone damages consisting of anterior rim erosion and bony avulsion are very important in decision making for treatment of recurrent dislocation in shoulder joint. This study was aimed to determine the prevalence of these damages in patients with anterior recurrent shoulder dislocation.   Methods : The study was a cross-sectional study evaluating patients with unstable shou...
